CPI Tools for Success: Addressing Modified Duty

Clinic Performance Initiative (CPI) Star Ratings are now published in the SelectNet directory, and you may be wondering how to improve your clinic scores. To do this, you must consistently follow the standards below for accurate evaluation of interim WC164 documents. Actual work status or types of restrictions are not considered when evaluating the WC164 forms in the context of CPI. The provision of specific information is the only consideration.Interim WC164 forms are evaluated for completeness. Ensure all information is legible and that modified duty information is provided at every visit, including:
- Work status in section 6b on the WC164 form
- Specific physical limitations in section 6c on the WC164 form
- Because specific limitations are required in section 6c, responses such as, “same”, “no change”, “see specialist report” or “as tolerated” are not accepted. Additionally, if an injured worker is placed at full duty and no restrictions apply, indicate “no restrictions”.
- A physician’s signature or printed name is required (signature or printed name of mid-level practitioner alone is not acceptable).
- Date of report field must be completed.
