Assessment involves asking attitudinal and factual questions. Attitudinal questions ask the injured workers for their opinions of their condition and factual questions ask workers to describe current and past abilities or limitations. This document includes introductory, return to work, and progress questions as well as questions about specific activities of daily living (ADL) such as driving, shopping, errands, lawn care, household chores, and personal hygiene.
