Z0756 — Independent medical exam (IME)
Fee schedule: $93.50 per unit
- Units are billed in 15-minute increments.
- Prepayment of two hours is allowed.
Cancellation before the IME
- If the cancellation is less than 14 days prior to the appointment, payment for all reasonable time, such as the preparation and scheduled appointment time, is allowed.
- If the cancellation is received over 14 days ahead, payment for all reasonable preparation time, less any deposit paid by the requesting party, is allowed.
Z0730 — Preparation for deposition and testimony
Fee schedule: $190.74 per unit
- Units are billed in 30-minute increments.
Z0734 — Deposition
Fee schedule: $190.74 per unit
- Units are billed in 30-minute increments.
- Prepayment of one hour is allowed.
Cancellation before the deposition
- If the cancellation is less than ten business days prior, payment for all reasonable time, such as the preparation and scheduled deposition time, is allowed.
- If the cancellation is received over ten business days ahead, payment for all reasonable preparation time, less any deposit paid by the requesting party, is allowed.
Z0738 — Testimony
Fee schedule: $264.18 per unit
- Units are billed in 30-minute increments.
- Prepayment of four hours is allowed.
Cancellation before testimony
- If the cancellation is less than ten business days prior, payment or all reasonable time, such as preparation and schedule testimony time, is allowed.
- If the cancellation is received over ten business days ahead, payment for all reasonable preparation time, less any deposit paid by the requesting party, is allowed.
